friends with benefits membership
Terms and conditions

1.  All membership requests must be submitted by completing the Friends with Benefits Interest Form. 

 

2. Membership applicants must be at least 18 years old.

3. Your membership is personal to you as the member and may not be transferred. However, you may gift one missed service per month with the exception of the Radiance Club. 

​

4. An initiation fee of $49.00 will be assessed at time of sign-up.

​

5. Monthly membership payments, or if paid in full, are non-refundable.

​

6. Facially Skin Bar reserves the right to change policies, services, and pricing  and make a change in facilities at any time without notice.

 

7. You must be a member for at least 3 months and have made at least three monthly membership payments (or have paid in full) in order to be eligible to freeze your membership. 

 

8. When a membership is frozen, the member is not authorized to use his/her membership services or benefits during the freeze period.

9. At the end of the membership freeze term, your dues will continue with your next scheduled electronic funds transfer date. 

 

10. If the membership is terminated or suspended after the Initial Period, reactivation must be renewed under a new agreement.

11. To initiate a freeze, submit a written request to Facially Skin Bar at fuelyourface@faciallyskinbar.com.

​

12. A 30-day written notice before your next scheduled payment is required to cancel a membership plan. No refunds will be issued after monthly membership due has been charged. 

​

13. Facially Skin Bar reserves the right to cancel or suspend the membership of any member for any reason.

 

14. Member will incur an administration charge of $30 to reactivate a membership.

 

15. Upon signing up, you (“the Member”) authorize Facially Skin Bar to charge your provided credit/debit card or bank account automatically on a recurring basis for your membership dues.

 

16. It is the Member’s responsibility to inform Facially Skin Bar of any changes to their payment met

 

17.​ Facially Skin Bar is not responsible for any injury or loss of property to any person while on the premises or participating in Facially Skin Bar’s services.​​

 

18. Your membership will remain in effect until:

 

(a) The Member cancels their membership.

 

(b) The Member’s payment method is no longer valid, and no alternative payment method is provided.

 

By joining our Membership Club, you acknowledge that you have read, understand, and agree to the policy terms outlined in this Agreement.
